The United Kingdom has once again lost its status as a measles-eliminated country, confirming sustained transmission of the highly contagious disease. This marks a cyclical pattern: the UK achieved elimination in 2016, faltered in 2018 alongside rising cases across Europe, regained status in 2021 (likely due to COVID-era hygiene measures reducing transmission), and now, in 2024, has reverted to endemicity.

The Erosion of Herd Immunity

The resurgence is not unexpected. Experts, like Ben Kasstan-Dabush of the London School of Hygiene and Tropical Medicine, point to a decade-long decline in routine vaccination coverage as the primary driver. The World Health Organization (WHO) recommends 95% coverage with two doses of the measles, mumps, and rubella (MMR) vaccine to achieve herd immunity. The UK currently stands below this threshold, with 92.3% coverage for the first dose and 84.4% for the second in 2024—numbers largely unchanged from 2023.

Why this matters: Measles is exceptionally infectious. Without high vaccination rates, the virus spreads rapidly, particularly among unvaccinated children. The decline in coverage isn’t uniform; areas like Hackney in East London demonstrate disproportionately low rates, exacerbating local risk.

UK Vaccination Policy and Trends

The UK’s National Health Service recommends the MMR vaccine (and, from 2026, the MMRV vaccine, which also protects against chickenpox) as part of its immunization program. Children are currently scheduled for doses at 12 and 18 months, though older schedules existed previously. Vaccination is not mandatory.

The trend is clear: Coverage peaked in 2016 (95.3% first dose, 88.2% second) and has steadily decreased since then. This decline isn’t just a statistic; it translates directly into increased vulnerability to outbreaks.

Global Context: A Wider Problem

The UK is not alone. Several other nations in the WHO European Region – Spain, Austria, Armenia, Azerbaijan, and Uzbekistan – have also lost their measles elimination status. Canada lost its status in November 2025, and the United States is on the verge of doing so, with ongoing outbreak investigations that could confirm year-long continuous transmission.
